reports without lessons learned.

sample of 22 problem closure reports, which shows that 8 incidents were caused
by IT Changes.

sample of 22 problem closure reports, which shows that 6 were due to
application-related issues.
4- These samples indicate that lessons learned from similar problems can increase the possibility of avoiding such instances in the future.
